Summary:
The Human Resources Department and Procurement and Contracts Division are requesting approval to extend the current contract with Symetra Life Insurance Company for one (1) additional year effective January 1, 2019 through December 31, 2019 (original effective date was January 1, 2015). The estimated 2019 annual premium amount is $377,000.
The negotiated Individual Advantage Deductible will increase from $281,490 to $334,300. All other terms of the current contract will remain unchanged from plan year 2018, including the premium amount of $6.35 per subscriber per month.
Benefit contracts are exempt from M/WBE participation requirements per City Code and M/WBE participation is not applicable to this contract (City Ordinance 57.15(4)). |
